Background: Insulin resistance plays a larger role in the T1D disease process than is commonly recognized. There are different mechanisms for its development in diabetic patients. DPP4 Inhibitors as a novel class of anti-diabetic agents were confirmed to improve glycemic control and preserve Ý-cell function in T2D. Several in vitro and in vivo studies are being conducted to detect whether DPP4 inhibitors have similar therapeutic efficacy in T1D. Objective: To assess the role of circulating DPP4 in the development of insulin resistance in adolescents with T1D. Methodology: It was a cross sectional case control study conducted in the outpatient clinic of diabetic e Metabolic pediatric unit (DEMPU), children hospital, Cairo university where 50 T1D adolescents following at the diabetes outpatient clinic and 80 healthy adolescents were included in the study. History taking and anthropometric measures (weight, height, BMI, waist circumference) and blood pressure measurement were initially performed, then the patients were assessed for the presence of insulin resistance using the equation for insulin sensitivity (eGDR). Biochemical evaluation for glycemic control ( HbA1c), lipid profile and serum level of DPP4 was done
