The usage ofbroncho-alveolar lavage (BAL) in the diagnosis of DPLD has been greatly reduced by the emergence of high resolution computed tomography (HRCT) Objectives: The purpose of this study was to optimize the role of broncho-alveolar lavage (BAL) in conjunction with high-resolution computed tomography (HRCT) for analyses of diffuse parenchymal lung diseases. This study is aiming to optimize BAL in association with HRCT and clinical findings to narrow down the cause of DPLD and thus, help in better management of the disease. Methods: This study was conducted in Chest department, Kasr Al Ainy hospital, Cairo University. Thirty twopatients with radiographic signs of diffuse parenchymal lung disease were selected for BAL. During flexible bronchoscopy, the probe was introduced into the selected bronchus. After general inspection of the tracheo-bronchial tree and before biopsy, the bronchoscope is brought into position with lavage of the selected area. The total volume of fluid was processed for cytology
