Background: Premenstrual syndrome (PMS) defined as a recurring pattern of physical, affective, and behavioral symptoms occurring during the luteal phase of menses and impair the daily lives of women. Aim: This current study was designed to detect the relation between body mass index and premenstrual syndrome. Subjects and Methods: this study was carried upon two hundred and forty females selected from students of faculty of physical therapy Kafr Elshiekh to participated in the study. Their age ranged from 18 to 22. They were divided into three groups according to body mass index; group (A): consist of 23 underweight females, group (B): 119 females with normal weight and group (C): 98 young females with overweight. Data were collected by instructing females to full fill recording data sheet and premenstrual syndrome scale questionnaire. Results: Mild severity symptoms of premenstrual syndrome scale were 4 (17.4%), 53 (44.5%), significantly high with (p-value < 0.0041), and 10 (10.2%), significantly low with (p-value < 0.0041), in group (A,B and C) respectively, while very severe severity symptoms of premenstrual syndrome scale were 6 (17.4%), 17 (44.5%), significantly low with (p-value < 0.0041), and 44 (10.2%), significantly high with (p-value < 0.0041), in group (A,B&C) respectively. Conclusion: There were positive relation between BMI and PMS
