Introduction: Eustachian tube (ET) dysfunction has been attributed to many diseases of middle ear cleft ranging from as benign as serous otitis media to squamosal (unsafe) chronic otitis media (COM). Many factors have been attributed for the success of tympanoplasty, out of which ET function (ETF) is considered one of the most important. ETF can be evaluated by various tests and assessment of the same can predict the outcome of tympanoplasty. Objective: The objective of this study was to evaluate the effect of ETF on tympanoplasty outcome in COM patients and detect the correlation between the type of the used graft and the outcome of tympanoplasty. Materials and Methods: This prospective study was carried on 41 patients (42 ears) diagnosed to have tubotympanic type of chronic suppurative otitis media. All patients underwent tympanoplasty type 1. Patients were randomly selected from outpatient clinics of Otolaryngology Department in Kasr El-Ainy University Hospital and hearing and speech institute from December 2016 to June 2018. Results: Out of 21 patients (22 ears) who had normal ETF, 18 (81.8%) had successful while four (18.2%) had unsuccessful outcome. Out of 20 patients who had ETD, 12 (60%) had successful while 8 (40%) had unsuccessful outcome. Out of 21 ears where Fascia grafts were used, 16 (76.19%) had healed while 5 (23.8%) had failed. Out of 21 ears where cartilage grafts were used, 14 (66.6%) had healed while 7 (33.3%) had failed
