Background: Tetralogy of Fallot, which is one of the most common congenital heart disorders. The mortality rate in untreated patients reaches 50% by age 6 years, but with the advent of improved medical and surgical care in congenital heart disease, there has been an increase in the number of patients who survive into adulthood, giving rise to a new patient population 2Adults with congenital heart disease3. And now, attention has been shifted to other factors such as psychosocial and cognitive functioning. Aim of work: Is to investigate executive function in adolescents and young adults with operated congenital heart disease (Tetralogy of Fallot) and compare the results to matched controls. Patients and methods: This study included 70 patients with tetralogy of Fallot and 70 healthy controls that had Wechsler Intelligence Scale for children (WISC IV) to assess their neurocognitive function. Results: The cognitive function of the cardiac patients Is significantly lower than the corresponding healthy control cases (P value =0.001) regarding their verbal (P value 0.006) and performance subtests (P value 0.020) and though the total score in WISC IV with statistical significance factors; BT shunt (P value= 0.040) and Post-operative ICU stay (P value= 0.006) and statistical insignificant factors; Pre-operative O2 saturation (P value= 0.472) and age at surgery (P value=0.543) Conclusion: Patients of TOF had post-operative cognitive dysfunction. Severe hypoxia and prolonged Post-operative ICU stay are contributing factors for cognitive brain function impairment
