Dissertation note: Thesis (M.Sc.) - Cairo University - Faculty of Medicine - Department of Internal Medicine Summary: Background: In chronic kidney disease (CKD) arginine vasopressin (AVP) cannot efficiently act via renal V2-receptors. AVP is upregulated leading to augmented activation of V1a- and V1b-receptors, which might contribute to the increase in cardiovascular and infectious complications in CKD. Here, we evaluated copeptin, as a surrogate marker of AVP, and its association with All-cause mortality among patients with end stage renal disease on regular hemodialysis. We aimed to measure Copeptin serially at baseline & 3 months and correlate with mortality. Methods: Copeptin was measured at baseline and 3 months in 48 patients with end stage renal disease on regular hemodialysis and 4 control subjects & comordibities were noted. Venous samples were withdrawn on the day of hemodialysis prior to the session. The copeptin levels were measured using an ELISA kits. Results: The levels of Copeptin were higher in patients with end stage renal disease on regular hemodialysis compared to control subjects(p<0.001).The baseline was correlated with mortality 3 patients died at 3 months, 0 patients died at 6 months. Serum copeptin was higher in patients with hypertension compared to non Hypertensive patients (0.002) & It was also higher in patients with IHD compared to those who don{u2019}t have IHD (p=0.017).Baseline serum copeptin level of 10.8 pmol/L had the a sensitivity of 60% and specificity of 90% for predicting 3 months mortality
