Background: Balance impairment is one of the primary problems in cerebral palsy. Ankle foot orthoses (AFO) is used in promising for improving function and stability. Purpose of the study: The aim of systematic review is to evaluate the quality of research investigating whether ankle foot orthosis is an effective rehabilitation tool on improving balance in children with spastic cerebral palsy. Methods: An extensive search on four databases(Cochrane Library ,pub-Med, PEDro and Web of scince) identified all study designs that include key words (Cerebral Palsy, Balance, Children, Ankle Foot Orthoses). A total of 357 papers were identified and only 12 met the inclusion criteria. Results: Six studies had low evidence base scoring less than 4 on Pedro scale while only four studies had high evidence base scoring more than 5 scoring on Pedro scale and two studies had moderate evidence base scoring 4. Conclusion: The analysis of the results of the included studies current studies revealed weak evidence in the effect of ankle foot orthosis on balance in children with spastic cerebral palsy
