Achieving restorations that are optimally aesthetic can be a technique sensitive due to previous guidelines on building complex layered composite restorations with suggestion to mix several shades and materials of varying consistency. Dentists commonly report that such techniques are time consuming, complicated, and don{u2019}t offer predictability in terms of aesthetic. Therefore, the direct composite placement technique became overwhelming, time consuming, and very important factor in the restoration success. The current in vivo study was conducted to evaluate the ability of two direct composite placement techniques to provide a restoration achieving patient satisfaction, fulfill shade match with the remaining tooth structure, and with good clinical performance.Materials & methods: In a split mouth design, 42 patients (84 restorations) of class IV or class III through and through within a pair of anterior contra-lateral teeth were randomly allocated by selecting a sealed envelope containing which half received either the (treatment 1) control; polychromatic layering or other (treatment 2) intervention; dual-shade layering. Materials were applied according to the manufacturers{u2019} instructions. Restorations were evaluated at baseline (one month) and after one year for patient satisfaction using Visual Analog Scale (VAS), restoration color shade match using Vita Easyshade V ®, and blinded assessor using FDI criteria for assessment of dental restorations measuring (aesthetic properties). A double blinded study was done as the patient did not know the treatment he is receiving, and the assessor was blinded about the treatment option given. The operator was not blinded because of the difference between steps of both techniques and protocol of the restorative materials application which prohibited blinding of the operator.
