Background: Apolipoproteins A1, B, and their ratio are recognized as a better indicatorsof cardiovascular disease than other cholesterol measures. Whether intracranial or extracranial atherosclerosis is more closely associated with an increased apolipoprotein B/A1 ratio has not been well established. Objective:This study aimed to assess the relation between apolipoprotein A1, apolipoprotein B, and their ratioto intracranial and extracranial atherosclerotic arterial stenosis, and to assess the difference in frequency of certain vascular risk factors between intracranial and extracranial atherosclerotic arterial stenosis in patients with acute ischemic stroke. Subjects and Methods: Ninety Egyptian patients with acute ischemic strokewere included in this study. They weredistributed into three equal matched groups; thirty patients had intracranial arterial stenosis (group I), thirty patients had extracranial arterial stenosis (group II) and thirty patients had non arterial stenosis (group III).All patients were subjected to complete clinical assessment, routine laboratory assessment including lipid profile and Color-Coded Duplex Sonography for extracranial and intracranial arteries. Measurement of serum levels of apolipoproteins A1, B using Enzyme Linked Immuno-Sorbent Assay (ELISA) was done for all patientsbiomarkers for cranial arterial stenosis in Egyptian patients with acute ischemic stroke
