Shoulder impingement syndrome (SIS) is a common diagnosis for patients with pain and dysfunction of the shoulder. Purpose of the study: To investigate the effect of adding core stability exercises program to the conventional physical therapy program in the treatment of shoulder impingement syndrome (SIS) regarding pain, disability and acromiohumeral distance. Material and methods: Thirty male and female patients were diagnosed as SIS, their age ranged between 25- 50 years were participated in this study. They would be randomly distributed into two equal experimental groups. Group (A): Received conventional physical therapy program. Group (B): Received core stability exercises combined with conventional physical therapy program. Patients in both groups received 12 sessions over four weeks, three sessions per week. The pain and disability were measured by Shoulder Pain and Disability Index (SPADI) and the acromiohumeral distance was measured at 0 degree and 45 degrees abduction, measured by digital goniometer. Results: There were significant increase in favor of group (B) in reducing pain, disability and increasing the acromiohumeral distance at rest with (00 shoulder abduction) secondary to reduction of internal structure size. There were significant increase in acromiohumeral distance in both groups with shoulder abduction at 450.Conclusion: adding core stabilization exercises to conventional physical therapy program in rehabilitation of SIS is very beneficial
