Dissertation note: Thesis (M.Sc.) - Cairo University - Faculty of Medicine - Department of E.N.T Summary: Synechia and scarring are the most common complications encountered after endoscopic sinus surgery (ESS) that may result in occlusion of the sinus drainage pathway leading to recurrent symptoms and subsequent surgical failure. In an attempt to avoid bleeding and prevent synechia formation, different techniques were suggested. Forty patient with bilateral sinusitis candidate forESS were encountered in the study. At the end of the procedure, the middle meatus and ethmoidal cavity at one side of the nose was irrigated using platelet rich plasma (PRP), while the other side (act as a control) was irrigated with normal saline according to a randomized assignment. Results showed that PRP side was more comfortable giving less pain, bleeding, nasal crustations and synechiaecompared to other side of the nose irrigated with saline (control) and that wasstatically significant. PRPcan improve mucosal healing as it has the ability to decrease synechiae, crust and edema. It also provide better quality of life for patient by decreasing postoperative pain and bleeding
