In the current study, we report our experience with the role of HE4 in diagnosis and preoperative staging of epithelial ovarian cancer (EOC). We collected and analyzed retrospective data of female patients presented with adnexal masses at National Cancer Institute (NCI), Cairo University (CU) over a period of one year from March 2018 till March 2019.The diagnostic performance of serum CA 125, HE4 and ROMA score in discriminating epithelial ovarian malignancy from benign pelvic mass was verified using the ROC analysis together with the AUC calculation for each marker For postmenopausal women, HE4 and ROMA score were statistically significant in diagnosis of EOC while CA 125 was statistically insignificant. Although for premenopausal women HE4, CA125 and ROMA score were statistically insignificant, AUC for HE4 still more than ROMA score and both more than CA125
