Background: Cardiovascular diseases and kidney diseases are closely interrelated. The detection of the preferable mode of dialysis, early recognition and appropriate therapy of myocardial dysfunction are desirable to prevent further progression to heart failure and cardiac mortality. 3D speckle tracking analysis allows assessment of myocardial deformation as new sensitive marker for detection of subclinical myocardial dysfunction. Objectives: The aim of this study was to evaluate the presence of myocardial dysfunction in patients undergoing acute peritoneal dialysis versus hemodialysis in cases of acute kidney injury and to assess of the preferable mode of dialysis on the cardiac tissue. Patients and methods: This is a retrospective cross-sectional case-control study included pediatric patients diagnosed as acute kidney injury that have undergone hemodialysis or peritoneal dialysis in the dialysis centre of pediatric nephrology unit (PNU) or intensive care unit (ICU) of Pediatric Hospital, Cairo University. It included 50 children; 25 of them were cases that suffered from acute kidney injury and have either undergone peritoneal dialysis or hemodialysis.They were compared to 25 healthy other children with age- and sex-matched as controls; randomly chosen from general clinic of Pediatric Hospital, Cairo University. Each patient was subjected to full history taking, clinical examination and echocardiography studies. Results: Our study shows that there was a statistically significant differences regarding all echocardiographic parameters between the cases and control group. This suggests that all the cases that have undergone dialysis, both peritoneal dialysis and hemodialysis, have cardiac dysfunction which were around 25 cases. There were statistically significant differences between the hemodialysis patients and the peritoneal dialysis patients regarding all of the echocardiographic measures
