Dissertation note: Thesis (M.Sc.) - Cairo University - Faculty of Medicine - Department of Orthopaedic Surgery Summary: Objective: the aim of this study is to compare between cast immobilization and volar locked plate for treatment of distal radius fracture AO classification type C in elderly patient in functional outcome as range of motion, grip strength and modified DASH score. Introduction:Distal Radius Fractures are common in the elderly. They account for more than 16% of all body fractures andthe incidence of this fracture is increased due to increasing number of elderly population along with increasing number of road traffic accidents and other high energy in all age groups who end up sustaining this fracture. There are numerous treatment modalities available to orthopedic surgeons in the treatment of a distal radius fracture; these include closed reduction which is accompanied by casting, percutaneous pinning or external fixation. Other modality is open reduction by volar or dorsal approach and internal fixation (by different implants such as volar distal radial locked plate or fragment specific plate). Patient and method:A total of 30 patients of distal end radius fractures AO classification type C divided into two groups, Group A included 15 cases which are treated by closed reduction and castingand Group B included 15 cases of operative management via volar locking plate.This studyis started from April 2019 to December 2019.All patients were followed prospectively 6 months. Results:Grip strength was superior with operative treatment of distal radius fracture type C by volar locked plate compared with conservative treatment with cast in patients greater than fifty years old with statistically significant between two groups of management
