Aim: This randomized clinical trial aimed to evaluate the clinical performance of direct fiber reinforced resin composite restorations versus direct microhybrid resin composite restorations in endodontically treated molars. Methods: In two parallel groups (n = 28), 56 participants having endodontically treated lower molars with moderate amount of remaining tooth structure were randomly enrolled in this trial and received either fiber reinforced resin composite (Ever X Posterior, GC, Japan) covered by microhybrid resin composite (Gaenial posterior, GC, Japan) restorations or microhybrid resin composite (Gaenial posterior, GC, Japan) restorations. Materials were applied according to the manufacturer instructions with universal adhesive (G-Premio BOND, GC, Japan). Two well trained experienced blinded assessors evaluated the restorations at baseline and 6 months using modified USPHS criteria. Staistical analysis: Mann-Whitney U test was used for intergroup comparisons and Wilcoxon signed rank test was used for intragroup comparisons.The significance level was set at p {u2264} 0.05. Results: No statistically significant difference was observed between both tested groups for all USPHS criteria at different follow-up periods except for color match favoring the microhybrid resin composite restorations with a significant difference between both groups (p= 0.002) at baseline and same after 6 months follow-up period
