Dissertation note: Thesis (Ph.D.) - Cairo University - Faculty of Medicine - Department of Orthopedics Surgery Summary: Comminuted subtrochanteric fractures of the femur are usually the result of high energy trauma and these present a challenge in management .Because of their unique anatomical features with bulky muscles acting on this area and the especial biomechanical properties due to the very high compressive and tensile stresses ,theses fractures are accompanied with high risk of complications such as malunion, nonunion, metal failure . In this study the union rate was 100% with no cases suffered infection. The results of this study show that Proximal femoral locked plate is a suitable implant for fixation of the comminuted subtrochanteric fracture femur in condition that we preserve the vascularity of the bone by biological fixation, achieve perfect placement of the plate along the proximal femur to prevent varusmalalignment and get adequate length of the plate to decrease the stiffness of the construct
