Introduction: Transfusion of blood and blood products is an important therapy for many patients; however, it still carries the risk of transmission of infection. Transfusion of Hepatitis B virus is common through blood transfusion .This highlights the importance of combined HBsAg and anti-HBc screening to eliminate blood-transmitted HBV. Aim of the work: The aim of this study was to screen the vaccinated multi-transfused patients in the outpatient clinic for hematologic disease at Aborish pediatric hospital, for HBV and to assess the prevalence of HBV among those patients.This way reflect the safety of the environment, medical procedures including blood transfusion and the immune status of those patients. Material and Method: The study was conducted on 805 multi-transfused patients who had been attending the outpatient pediatric hematology clinic. Plasma blood samples were collected for HBV rapid screening test and serum samples were collected for serological and molecular analysis of positive cases. Results: Our results of HBV screening using RDT revealed 3 positive cases out of 805. The first case showed HBsAg positive, HBsAb was negative (2mIU/ml), HBcAb positive, HBeAg was positive and high viral load.The second case showed showed HBsAg positive, HBsAb negative (2mIU/ml), HBcAb positive, HBeAg was negative and intermediate viral load. And the third case showed showed HBsAg negative, HBsAb was positive (20mIU/ml), HBcAb positive, HBeAg was negative with undetectable titer
