Background Rheumatoid arthritis and systemic lupus erythematosus are autoimmune diseases with articular and extraarticular manifestations. Premature atherosclerosis and cardiovascular diseases are two of the most important complications of these diseases.Several markers and antibodies were present for detection of these diseases. Anti-carbamylated protein antibody (Anti-carP) is one of the antibodies which were studied in rheumatoid arthritis especially as a predictor of development of erosions. The relation of this antibody with disease activity or insulin resistance wasn't studied in both diseases. In our study, we studied the relation between anti-carbamylated protein antibody and disease activity and insulin resistance in rheumatoid arthritis (RA) and systemic lupus erythematosus (SLE) patients. Methods 90 Patients with SLE and RA were enrolled.They were subjected to full history taking and clinical examination with assessment of disease activity using SLEDAI-2K scoring for SLE patients and DAS28-ESR scoring for RA patients. Samples were extracted and examined for complete blood count, creatinine, inflammatory markers, tumor necrosis factor alpha (TNF alpha), fasting insulin and fasting blood sugar, lipid profile and anti-carp antibody. HOMA-IR (homeostasis model assessment for insulin resistance) was calculated. Results 90 patients were enrolled. Patients with RA and SLE had higher levels of anti-carp antibody in comparison with healthy subjects (8.25 ng/ml for RA, 7 ng/ml for SLE and 0.6 ng/ml for the healthy subjects with p value <0.001)
