Background and purpose: Hematopoietic stem cell transplantation (HSCT) is a potentially curative therapeutic modality used primarily in the treatment of hematological malignancies and bone marrow failure syndromes, The annual number of such transplantations continues to rise, with >8,000 allo-HSCT and >14,000 auto-HSCT procedures performed in the USA in 2018.Objectives: The purpose of this study was to determine the impact of moderate aerobic exercise on Functional capacity, IL-6 levels and quality of life (Mobility, Breathing and Usual activities) after HSCT.The study was conducted for three months, three sessions per week 40 minutes each session. Methods: This study was conducted on forty women their age's ranges from 20 to 30 years old and their body mass index (BMI) ranges from 18.5-24.9 selected from Nasser Institute Bone Marrow Transplant Clinic and Belqas Central Hospital as follow up.They were assigned into two groups equally in numbers: Study Group: received moderate intensity aerobic exercise on treadmill or stationary bicycle (score 12-14 on Borg scale for rate of perceived exertion) plus their routine medical regimen. Control Group: They were only follow their own medical regimen. Results: Mobility scores were significantly lower in patients who exercised (median: 1.0 vs. 2.5 and the percent of change was -44.167%). Similarly, breathing and usual activities scores were significantly lower in patients who exercised (breathing; 2.0 vs. 3.0 with percentage of change by -30.0%, usual activities, 2.0 vs. 3.0 with -21.667% change), all indicating general improvement of quality of life scores in the exercise group. After training programm 6 MWT were significantly higher in the exercise group (317.2 m vs. 272.8, and the percent of improvement was 18.629% )
