Dissertation note: Thesis (M.Sc.) - Cairo University - Faculty of Medicine - Department of Pediatrics Summary: Introduction: Type 1 diabetes mellitus (T1DM) is a complex chronic condition of childhood resulting from gradual autoimmune destruction of the insulin producing pancreatic islet cells. Obesity is a well{u2010}known risk factor for insulin resistance and adds to the difficulty of the management of youth with T1DM. Additionally hypoadinectinemia is independently associated with metabolic syndrome (MS) as a marker of IR, and significantly related to type 2 diabetes (T2D) development. Aim of study: to determine the serum level of adiponectin and its relation to insulin resistance in children and adolescents with T1D. Patients and methods: This was a cross sectional study which included sixty five (n=65) children diagnosed with T1D who were presented and followed up at Pediatric Endocrinology Department at Cairo University Children{u2018}s Hospital from November 2020 to January 2021. All patients were subjected to full history taking; clinical evaluation and laboratory results were obtained from files, in addition to serum level assessment of adiponectin. Results: 40% of study group were hypertensive, about 40% of the study group were pubertal. 16.9% had signs of poor metabolic control in the form of limited joint mobility. Most of children (82.4%) in the present study were poorly controlled as the mean HbA1c was 9.6±1.5. The mean value of estimated glucose disposal rate was 6.9±2.1 with median 7.2 and range between (1.7-10.7). And 93.8% of diabetic patients were dyslipidemic. About 40% of diabetic patients had low serum adiponectin with the mean value 2.4±3.6
