Dissertation note: Thesis (Ph.D.) - Cairo University - Faculty of Medicine - Department of Radio-Diagnosis Summary: Background: Characterization of borderline ovarian lesion represents a diagnostic challenge; it is of great importance in the preoperative assessment in order to plan adequate therapeutic procedures especially in women in childbearing period aiming to preserve fertility. Objective: to evaluate the diagnostic performance of ultrasound, Doppler and multiparametric MRI in the assessment of borderline ovarian masses. As well as detection of different significant diagnostic tools in different imaging techniques. Methods: fifty seven complex adnexal lesions fulfilling ORADS 3 and 4 criteria with high suspicion of being borderline rather than malignant or benign either by ultrasound, MRI or both. Six lesions were pathologically proven non ovarian origin. 51 ovarian lesions were included in the final statistical analysis. Preoperative ultrasound , MRI and CA-125 level measurement then surgical interference were done to all the included cases. Finally pathological correlation was done. Results: The fifty one ovarian lesions were classified by pathology into 29 borderline, 12 malignant and 10 benign lesions. Regarding the demographic data, younger age and premenopausal status were significantly observed in BOT patients. Large lesions were associated more with malignant rather than borderline lesions (p=0.07). Complex cystic lesions with smaller papillary projections as regards the size and = or <3 regarding the number, were significantly seen in BOT compared to malignant lesions
