Background Bariatric surgery is a treatment for morbid obesity. Different surgical procedures have been described in order to obtain excess weight loss (EWL), but currently laparoscopic sleeve gastrectomy is the most commonly performed procedure throughout the world. Reducing abdominal wall trauma and increasing the aesthetic result are important goals for all bariatric surgeons.The aim of the study is to assess and compare the practicability and efficacy of laparoscopic three-port sleeve gastrectomy (3PSG) and conventional laparoscopic sleeve gastrectomy (LSG). Method A prospective randomized comparative study between patients underwent 3PSG (group 1) and conventional LSG (group 2), each containing 50 patients after application of the inclusion and exclusion criteria. Results Mean operation time was longer in group1 (50±4.38 min vs. 46.04±3.64 min, p < 0.001), but patient satisfaction was better in the three-trocar sleeve gastrectomy group. (p < 0.001) The median VAS was 3.38±1.38 and 3.68±1.45 in group 1 &2 respectively (p > 0.05). EBWL% was similar in both groups.There were no intraoperative complications or major complication as leakage or bleeding in both groups. No cases needed an extra port in 3PSG group.There was no clinically apparent trocar site hernia
