Dissertation note: Thesis (M.Sc.) - Cairo University - Faculty of Medicine - Department of Radio-diagnosis Summary: Aim of work: Renal biopsy is the gold standard for the histological characterization of chronic allograft nephropathy (CAN), of which renal fibrosis is a dominant component, affecting its stiffness.The aim of this study is to investigate the correlation between renal allograft stiffness obtained by shear wave elastography (SWE) and renal histological fibrosis. Methods: Shear wave elastography assessments were performed in 18 CAN patients who underwent renal biopsy.The SWE-derived mean value of the tissue stiffness, given as kilopascals (kPa), was measured. Mean tissue stiffness value was correlated to patients' renal histological scores of interstitial fibrosis and tubular atrophy (IF/TA). Results: SWE-derived mean kidney stiffness value correlates significantly with tubule-interstitial score with sensitivity 87.5%, specificity 90% and p value >0.001. The mean kidney stiffness value increased as the percentage of interstitial fibrosis and tubular atrophy increased. The area under the receiver operating characteristic curve (ROC) for SWE in differentiating between mildly and moderately impaired kidneys was 0.91. Conclusion: Shear wave elastography can accurately detect moderate chronic allograft nephropathy resulting from interstitial fibrosis and tubular atrophy, using the optimal cut-off value of 28.67 kPa
