Dissertation note: Thesis (M.Sc.) - Cairo University - Faculty of Medicine - Department of Gynecology and Obstetrics Summary: Background: An early abortion is the termination of a pregnancy during the first trimester (the first 3 months of pregnancy). Early abortions can be accomplished with medication or surgery. Early abortions require less care and follow-up and are significantly more affordable than later abortions. Objective: Comparison between therapeutic efficacy of prostaglandins versus a combination therapy of prostaglandins and isosorbide-5-mononitrate in induction of cervical ripening and effacement for induction of the first trimester missed abortion. Methods: A randomized clinical trial was conducted and consisted of 60 women who had first trimesteric missed abortion and visited Embaba general hospital at obstetrics and gynecology department starting from December 2020 till June 2021. Cases were divided into two equal groups. Group A received only vaginal misoprostol and group B received combined vaginal misoprostol combined with isosorbide-5-mononitrate. Results: The duration till abortion was significantly shorter in group B.There was insignificant difference between both studied groups regarding side effects of study medications. There was insignificant difference between both studied groups as regards women{u2019}s satisfaction. Conclusion: Vaginal administration of isosorbide-5-mononitrate combined with misoprostol could be more effective and makes a synergistic action than using misoprostol alone in first trimester abortion
