Purpose. To ssess role of transpedicular screw fixation in treatment of cases of failed back surgery syndrome without instability. Methods. thirty patients of both genders aged from 28 to 60 years old with failed back surgery syndrome without instability, and after failed medical treatments and interventional techniques were included in the study. They were operated upon by transpedicular screw fixation, and followed up for at least six months. Results. 16 patients (53.3%) improved, 12 (40%)persisted pain and 2 (6.7%) worsened. Conclusion. Transpedicular screw fixation could be valuable in treatment of failed back surgery syndrome without instability when all other nonsurgical modalities have failed to releve manifestations
