Background: Preterm birth and its complications are now the leading cause of mortality in under 5 years children globally. Prolonged stay in NICU imposes emotional and economic burdens on the family and society. This study was carried out to estimate the length of stay of preterm neonates in NICU, Cairo University hospital and to identify neonatal and maternal factors associated with prolonged neonatal intensive care unit stay.Patients and methods: This retrospective cross-sectional study was conducted on 350 preterm neonates born at gestational age 26-37 weeks and admitted to NICU-Cairo university hospitals from January 2019 till January 2020. The tool used in this study was a checklist. The checklist was designed in English after reviewing the literature and the patients' records. Data collected from the records were computerized and analysed using SPSS version 18.Results: Results of the study showed that the mean length of stay in NICU is found to be 21.25 ± 17.02 days. There was a significant negative correlation between length of NICU stay and the following factors: birth weight, gestational age, neonates delivered by CS, mechanical ventilation and positive findings in chest x ray or echo.Prolonged stay in NICU was also noticed in cases of positive sepsis markers, bronchopulmonary dysplasia, necrotizing enterocolitis and intraventricular hemorrhage. Conclusion: the overall findings of the study showed that several neonatal and maternal risk factors affect the length of stay of premature neonates in NICU. Implementing different interventions can be effective on decreasing length of stay in NICU and improving neonatal survival rate
