Many methods were invented for ACL reconstruction , these methods varied in types of graft (synthetic and natural) , autografts or allograft , the autografts gave better results than synthetic or allograft, and between different types of autografts the hamstring tendons gave best results with minimal graft site morbidity . Many methods of fixation were used (cortical screws, threads, washers, interference screws, mini plates,) The interference screw method considered as the gold stander methods of fixation that also used to compare it and the newly invented methods, but this interference screw had its own problems like, screw breakdown, it is a foreign body, bone tunnel dilatation, it interfere with MRI and CT if it is needed postoperatively, also the revision ACL surgery became difficult with screws in addition to the financial side. A biological method of fixation were developed to fix the ACL graft using a bone plug that fix the graft by press fit technique , it has many advantages like it is a biological method no foreign body , decrease bone tunnel dilatation , it didn’t interfere with MRI or CT postoperatively , easy revision surgery , in addition to low coast. In this study we compare between the press fit technique and interference screw technique from clinical point of view to determine whether this method is effective and valuable or not. This a comparative prospective study done on a sample of population (30) patients, 15 patients fixed by interference screws and 15 patients fixed by press fit technique, and after collection of the data and statistical studies, we compare between the 2 techniques and between this study and similar studies, we found that the results are comparable in both groups (with some superiority of press fit technique in some clinical aspect ) in addition to the advantages of press fit technique that mentioned formerly, but from our clinical point of view this technique was technically demanding and has a less steep learning curve other than the ordinary well-known techniques in ACL reconstruction.
