<br/><br/> Central nervous system (CNS) involvement is<br/>a challenge in diagnosis & treatment. Diagnosis and treatment of central<br/>nervous system (CNS) infiltration and/or CNS relapse in childhood acute<br/>lymphoblastic leukemia (ALL) remain challenging. CNS leukemia is mainly<br/>a leptomeningeal disease, but leukemic cells may be detectable in the<br/>cerebrospinal fluid.The study aimed to assess the relation between CD79a<br/>expression on BM sample and overall, EFS and CNS involvement in B-cell<br/>ALL (ALL). Patients and methods: A retrospective study of expression of<br/>CD79a in 331 patients with pediatric B cell ALL, treated according to St. Jude<br/>total XV protocol at National Cancer Institute, Cairo University between<br/>January 2012 to December 2018. Results: the incidence of CNS involvement<br/>initially, there was 10 (3%) patients (CNS III),median value of CD79a<br/>expression in CNS III was as the following: 85.5 % ,MFI:140.0 MFI-ISO:<br/>128.0 MFI\ISO: 10.6 , while in (CNS I) was as the following, 42.0 % ,<br/>MFI: 74.0 , MFI-ISO:59.0, MFI\ISO:4.8. Regarding the CNS relapse, There<br/>is a strong relation between the degree of expression of CD79a in initial BM<br/>sample and the site of relapse, in isolated CNS and combined relapse showed<br/>higher level of expression of CD79a than hematological relapse. In isolated<br/>CNS relapse the median percentage of expression: 85, MFI:190,<br/>MFI\ISO:95,MFI-ISO:177,in combined relapse median percentage of<br/>expression:89,MFI:185,MFI\ISO:17.6,MFI-ISO:173 ,While hematological<br/>relapse:67,MFI:86,MFI\ISO:5.2,MFI-ISO:9.5. There is a Signiant impact of<br/>qualitative and qualitative expression of CD79a and the OS of Population<br/>who expressed percentage below(43.0) showed lower OS than patients who<br/>expressed above (43.0), P value ( <0.001), As regard MFI of expression of<br/>CD79a on initial BM sample , we have calculated it with cut point (77) the<br/>mean value of the expression on the whole cohort. It is significant impact on<br/>XII<br/>the OS , population who MFI below(77) showed lower OS than patients who<br/>expressed above (77), P value ( <0.001), As regard MFI\ISO of expression<br/>of CD79a on initial BM sample, we have calculated it with cut point (4.8) the<br/>mean value of the expression on the whole cohort. Conclusions: There is<br/>direct relation and impact on OS & EFS between quantitative CD79a on BM<br/>on CNS and site of relapse.  سرطان الدم اللمفاوى الحاد هو اكثر انواع سرطان الاطفال شيوعا و هو الانت يشهد نسبا كبيرة من الشفاء نتيجة لتصنيف العلاج علة اساس علم الوراثة الخلوية و الجزيئية و التنميط المناعى <br/>سرطان الدم اللمفاوى الحاد مسؤول عن 25% من جميع الاورام الخبيثة لدى الاطفال و 75% من سرطان الدم لدى الاطفال
