Central nervous system (CNS) involvement is
a challenge in diagnosis & treatment. Diagnosis and treatment of central
nervous system (CNS) infiltration and/or CNS relapse in childhood acute
lymphoblastic leukemia (ALL) remain challenging. CNS leukemia is mainly
a leptomeningeal disease, but leukemic cells may be detectable in the
cerebrospinal fluid.The study aimed to assess the relation between CD79a
expression on BM sample and overall, EFS and CNS involvement in B-cell
ALL (ALL). Patients and methods: A retrospective study of expression of
CD79a in 331 patients with pediatric B cell ALL, treated according to St. Jude
total XV protocol at National Cancer Institute, Cairo University between
January 2012 to December 2018. Results: the incidence of CNS involvement
initially, there was 10 (3%) patients (CNS III),median value of CD79a
expression in CNS III was as the following: 85.5 % ,MFI:140.0 MFI-ISO:
128.0 MFI\ISO: 10.6 , while in (CNS I) was as the following, 42.0 % ,
MFI: 74.0 , MFI-ISO:59.0, MFI\ISO:4.8. Regarding the CNS relapse, There
is a strong relation between the degree of expression of CD79a in initial BM
sample and the site of relapse, in isolated CNS and combined relapse showed
higher level of expression of CD79a than hematological relapse. In isolated
CNS relapse the median percentage of expression: 85, MFI:190,
MFI\ISO:95,MFI-ISO:177,in combined relapse median percentage of
expression:89,MFI:185,MFI\ISO:17.6,MFI-ISO:173 ,While hematological
relapse:67,MFI:86,MFI\ISO:5.2,MFI-ISO:9.5. There is a Signiant impact of
qualitative and qualitative expression of CD79a and the OS of Population
who expressed percentage below(43.0) showed lower OS than patients who
expressed above (43.0), P value ( <0.001), As regard MFI of expression of
CD79a on initial BM sample , we have calculated it with cut point (77) the
mean value of the expression on the whole cohort. It is significant impact on

the OS , population who MFI below(77) showed lower OS than patients who
expressed above (77), P value ( <0.001), As regard MFI\ISO of expression
of CD79a on initial BM sample, we have calculated it with cut point (4.8) the
mean value of the expression on the whole cohort. Conclusions: There is
direct relation and impact on OS & EFS between quantitative CD79a on BM
