After CABG respiratory therapy is used rationally to inhibit the post-operative complications. The removal of respiratory secretions is an important concern during the rehabilitation phase after cardiac surgery. The ability to perform deep inspiration and cough effectively, to prevent accumulation of secretions and improved gas exchange, is adversely affected by pain and postoperative apprehension associated with changes in lung mechanics resulting from surgery which eventually can lead to atelectasis, pneumonia, and increased mortality. Oscillating PEP (OPEP) therapy (Quake device) provides the combination of positive expiratory pressure with high frequency oscillations. It involves breathing with a slightly active expiration against an expiratory resistance through a device. It’s used for clearance of excess secretions from the lungs & reducing gas trapping and improving ventilation of the lungs The purpose of this research was to evaluate the efficacy of intrathoracic oscillations via quake device on ABG in post CABG patients. Sixty men patients underwent CABG in the 55-65 age range were included in this study. They were recruited from 6 October Hospital. All participants gave informed permission. C Summary  78 Patients were randomly assigned into 2 equal groups (30 patients for each group) with pre and post treatment protocol application. Both treatments were performed once daily from 1st day till 7th day after operation. Group A (Study group): Thirty men patients were treated by Oscillatory PEP (Quake device) along with traditional and prescribed medical treatment. Group B (Control group): Thirty men patients were treated by traditional treatment along with their prescribed medical treatment. Arterial blood gases, sit to stand test and SF-36 questionnaire were performed before and after treatment protocol application to assess the patient’s oxygenation status, functional capacity and health-related quality of life. The results of this study demonstrated a statistically significant difference between pre and post treatment values in all measured variables within quake device group. Results also revealed a significant difference in the favor of post treatment on measured variables compared to pre-treatment condition within control group. Comparing the two groups using the same measures revealed a statistically significant difference between them, with the group that received treatment by Quake device show a greater improvement than control group ; شمل هذا البحث ستون مريضا بعد اجراء جراحة توصيل الشرايين التاجية تراوحت اعمارهم ما بين خمس وخمسون وخمس وستون عاما تم اختيارهم من مستشفي 6 اكتوبر تم تقسيمهم الي مجموعتين المجموعة ( أ ) تضمنت المرضي الذين تم علاجهم بالوسائل العلاجية التقليدية بالإضافة الي استخدام جهاز (َQuake) والمجموعة (ب) والتي تضمنت المرضي الذين تم علاجهم بالوسائل التقليدية فقط. كلا المجموعتين تلقوا العلاج بشكل يومي ابتداء من اليوم الاول وحتي اليوم السابع بعد اجراء العملية وقد تم تقييم الحالات عن طريق اجراء تحليل غازات الدم واختبار الجلوس ثم الوقوف و تم ملئ استبيان SF 36. التأهيل المبكر باستخدام جهاز (Quake) بعد جراحة توصيل الشرايين التاجية يمكن ان يؤدي الي تحسن في غازات الدم والكفاءة الوظيفية مما ينعكس علي جودة الحياة للإنسان ER -
