Purpose: to compare the effect of iontophoresis versus ultrasound (US) on knee osteoarthritis. Methods: Seventy-two subjects diagnosed with KOA according to the American college of rheumatology (grade 2 and 3) participated in this study. Their age ranged from 45 to 60 years old while their BMI ranged from 19 to 30 kg/m2, They were allocated into three equal groups; Group A (experimental group) received iontophoresis and traditional exercises, Group B (experimental group) received US and traditional exercises and Group C (control group) received only traditional exercises . All patients received 12 session on alternative days. Pain intensity, knee ROM, and functional disability, were assessed using the VAS score, electronic digital goniometer, and the WOMAC index – Arabic version respectively. Results: There was a significant decrease in VAS of group A compared with that of group B (p = 0.02) and group C (p = 0.001), There was a significant increase in flexion ROM of group A compared with that of group B (p = 0.001) and group C (p = 0.001), There was a significant increase in extension ROM of group A compared with that of group B (p = 0.001) and group C (p = 0.001), There was a significant decrease in pain score of group A compared with that of group B (p = 0.004) and group C (p = 0.001),There was a significant decrease in stiffness score of group A compared with that of group B (p = 0.02) and group C (p = 0.001) and There was a significant decrease in function score of group A compared with that of group B (p = 0.009) and group C (p = 0.001).Conclusions: Iontophoresis combined with traditional exercises, US combined with traditional exercises and traditional exercises have good clinical outcomes regarding reducing pain intensity, increasing knee ROM, and improving functional disability with a greater superiority for the iontophoresis for patients with KOA.; شارك في هذه الدراسة اثنان وسبعون مريضاً، تم تشخيصهم بخشونة الركبة طبقاً للكلية الأمريكية للروماتيزم (الدرجة 2 و3). وقد تراوحت أعمارهم بين 45 إلى 60 عاماً، بينما مؤشر الكتلة إلى الجسم تراوح بين 19 و30 كجم/م2. تم توزيع المرضى على ثلاث مجموعات متساوية; المجموعة أ تلقت الارحال الايوني بالإضافة إلى تمارين تقليدية . المجموعة ب تلقت الموجات فوق الصوتية بالإضافة إلى تمارين تقليدية. المجموعة ج تلقت تمارين تقليدية. وقد تلقى جميع المرضى 12 جلسة علاج طبيعي على أيام متناوبة. وقد تم قياس شدة الألم، المدى الحركي للركبة، والإعاقة الوظيفية باستخدام المقياس البصري التناظري، مقياس الزوايا الرقمي الإلكتروني، ومؤشر التهاب المفاصل في جامعة ويسترن أونتاريو وجامعة ماكماستر ER -
