Retro walking is a novel treatment method for knee osteoarthritis. Aim of study: The study was conducted to investigate the effect of 4 weeks of retro-walking exercise in addition to conventional exercises on psychological status (primary outcome), pain, and knee joint function (secondary outcomes) in patients with knee osteoarthritis. Subjects and Methods: This study was carried out on forty-two patients of both genders with mild to moderate knee osteoarthritis. Their ages ranged from 40 to 55 years and their body mass index (BMI) ranged from 18.5 to 28.6 kg/m2. Patients were divided randomly into two groups: (group A) the control group received four weeks of conventional exercises three times per week and (group B) the experimental group received four weeks of retro walking exercise 3 times per week from 10 min up to 30 minutes added to conventional exercise, the psychological status was assessed by using the Arabic version of the Pain Anxiety Symptoms Scale (PASS) and Beck Depression Inventory-Ⅱ (BDI -Ⅱ), the intensity of Pain measured by the Arabic version of Visual Analogue Scale (VAS) and Knee joint function by using Arabic version of the WOMAC questionnaire. Results: The results of this study revealed that there was no significant difference between both groups in psychological status. There was a significant difference between both groups in pain and function in favor of group (B).Conclusion: Retro walking exercise added to conventional exercises is an effective method in relieving pain and improving function in osteoarthritis patients so it can be considered as part of the management of knee osteoarthritis.; اجريت التجربة على أجريت الدراسة على اثنين واربعين شخصًا من مرضى الالتهاب العظمى المفصلى للركبة (استقبل احدى وعشرون مريض تمارين معتادة واحدى وعشرون شخص استقبلوا تمارين معتادة مع تمرين المشى الخلفى). تراوحت اعمار المشاركين بين40-55 سنه، متوسط عمرهم(46.3±4.1) و(45.5±4). تم تسجيل التغير في مستوى الالم عن طربق مقياس فاس للالم( VAS) والاداء الوظيقى عن طريق استبيان ووماك (WOMAC) ,والتغيرات فى االحالة النفسية عن طريق مقياس الاكتئاب الشخصى بيك BDI -Ⅱ)) واستمارة باس (PASS) قبل البدء بالجلسات وبعد 12 جلسة لكلى المجموعتين. 0.05. ومن نتائج هذا البحث يمكن استخلاص أنه لا يوجد اختلاف بين المشى الخلفى لمعالجة الحالة النفسية المصاحبة للالتهاب العظمى المفصلى للركبة والتمارين المختارة ولكن هناك اختلاف ملحوظ على الالم والاداء الوظيفى لصالح المشى الخلفى ولذلك ينبغي النظر في ادراج المشى الخلفى ضمن خطة العلاج الخاصة لمرضى الالتهاب العظمى المفصلى للركبة ER -
