Accurate reconstruction of comminuted and old ZMC fractures is very challenging to achieve good esthetic and functional outcome. This study was designed to compare the computer assisted technique versus the conventional technique v in the reconstruction of the zygomatico-maxillary complex fracture .Fifteen patients with unilateral old ZMC were included and divided in three groups:- Patients were divided equally into three groups: Group I: Received computer assisted reconstruction of the ZMC guided by preformed acrylic surgical stent after mobilization of the ZMC. Group II: Received computer assisted ZMC reconstruction using prebent titanium mesh filled with autogenous bone graft without mobilization of the malunited ZMC. Group III: Received conventional (free hand) manual reduction & fixation after mobilization of the ZMC. Pre- and immediate post-operative high resolution multislice CT was ordered
