The combination of tramadol and alcohol caused threatening or fatal side effects. The aim of present study was to investigate the effect of ethanol, tramadol and there combination on rat liver and the role of grape seed extract (GSE) and silymarin as natural hepatoprotective agents versus ethanol-tramadol combination. Ten equal groups (8 rats each) were treated as follows: Group 1 kept as control group orally given 10ml / Kg, distilled water Groups 2, 3 and 4 treated with combination of ethanol (3.7g / Kg, P.O.) and tramadol (20 mg / kg, i.p.), tramadol (20 mg / kg) and alcohol (3.7g / kg), respectively. Groups 5 and 6 given GSE (150 mg / kg) in concomitant with tramadol and alcohol or with silymarin (100 mg / kg), respectively. Groups 7 and 8 administered GSE or silymarin concomitant with tramadol. Groups 9 and 10 orally treated with GSE or silymarin concomitant with alcohol
