Background Ultrasound imaging before neuraxial blocks was reported to improve the ease of insertion and minimize the traumatic trials. However the data about the use of ultrasound in thoracic epidural block are scanty. In this study we have done a randomized controlled trial comparing pre-insertion ultrasound scanning with traditional manual palpation technique. Results Ultrasound group showed lower number of puncture attempts (1.5(1, 2.75) Vs1(1, 1.25) , P=0.008 ), puncture levels (1(1, 2) Vs1(1, 1) , P=0.002 ), needle redirection (3.5(2, 5) Vs0(0, 2.25) , P=0.00 ), and shorter time(153±38 VS 216±78, P=0.00) than manual palpation group. Conclusion Pre-procedural ultrasound imaging improved the ease of insertion of thoracic epidural catheters compared to manual palpation method.
