Rapid on - site examination of transbronchial needle aspirates has long been used during flexible bronchoscopy, but its usefulness in the diagnosis of hilar and mediastinal adenopathy is controversial. The aim of the present study is to evaluate the extent to which ROSE can be valuable in patients undergoing transbronchial needle aspiration for the diagnosis of hilar and mediastinal adenopathy. ROSE of transbronchial aspirates from hilar and mediastinal nodes enables avoidance of additional biopsy without loss in diagnostic yield and reduces the complication rate of bronchoscopy. To determine the effect of ROSE on the diagnostic power of TBNA. The present study included 20 patients subjected basically to written informed consent, full history taking, full clinical examination, chest x-ray & CT chest, fiberoptic bronchoscopy with transbronchial needle aspiration (either blind or endobronchial ultrasound guided), Rapid on site examination of the aspirates to detect the presence of lymphoid tissue, the final diagnosis in each patient would be established by correlating the pathologic findings with the clinical - radiologic data. The study included 13 males (65%) and 7 females (35%) with mean age 47.75 ± 13.026. 12 patients (60%) were diagnosed as malignancy, 7 patients (35%) were diagnosed as Sarcoidosis and one patient (5%) was diagnosed as T.B
