Hepatocellular carcinoma (HCC) is the commonest primary liver cancer (80 - 90%) and represents 4% of all cancer deaths and the 5th cause of death from cancer according to latest statistics according to western population. Chronic infection with hepatitis C virus (HCV) is one of the common factors for HCC occurrence. HCV infection causes oxidative stress in hepatic cells through overproduction of reactive oxygen species (ROS) that cause carcinogenesis. The presence of liver cirrhosis is an important factor that affects the prognosis of patients with (HCC). Cirrhotic patients with a high model for End - stage Liver disease (MELD) or late complication as malignancy. About 45%-50% of cases with hepatocellular carcinoma demonstrate FDG uptake higher than the liver parenchyma, whereas 50% - 55% of cases show FDG uptake similar to that of liver parenchyma. The present study is retrospective analysis for assessment of the role of FDG - PET in the initial diagnosis of hepatocellular carcinoma and presence of extra hepatic metastases
