Background: Sudden Sensorineural hearing loss (SSNHL) is a sensorineural hearing loss of greater than 30 dB over 3 contiguous pure tone frequencies occurring within 3 days period , The development of vascular damage requires dynamic interaction between leukocyte surface receptors and their ligands on the endothelial cell surface, The hypothesis that vascular damage might have a pathogenic role in immune- mediated SSNHL is supported by labyrinthine fibrosis and cochlear ossification found in those patient , Elevated levels of circulating adhesion molecules have been detected in disorders where leukocyte/endothelial cell interaction play a significant role, such as inflammatory diseases and vasculitis, and have been considered a consequence of endothelial and/or immune activation , Materials and method:The present study included 40 patients with idiopathic SSNHL (ISSNHL), 20 healthy subjects, age and sex matched, were included in the study as a control group, Some immunological parameters, namely erythrocyte sedimentation rate (ESR), platelets(PLT),Total Leucocytic Count (TLC),Anti-Endothelial Cell Antibodies( AECA), E.selectin and ICAM-1 were done for patients and control groups, Aim of work :The aim of this study is to verify if the vascular damage could have a role in the pathogenesis of SSNHL and to evaluate the role of anti endothelial antibodies (AECA) ,intercellular adhesion molecule-1 (ICAM-1) and E-selectin as serological markers of vasculitis in patients affected with SSNHL. Results: ESR, ICAM-1 and AECA levels showed highly significant elevations in patients group as compared to controls,These Parameters were significantin profound cases than severe and moderate Thus these parameters could be of value in suspecting immune-mediated vasculitis in hearing loss, TLC and PLT levels showed highly significant elevations in patients group as compared to controls with no significant difference between severities among cases group, E.selectin show no significant difference between cases and control group and no significant difference between severities among cases group,
