Methods: prospective randomised clinical study study of 30 eyes of patients of DME underwent PPV divided into 2 equal groups, group A and group B. Group A: underwent PPV without ILM peeling. Group B: underwent PPV with ILM peeling. Results: It was found that the CMT was improved in 10 cases (66.67%) in group A compared to 12 cases (80%) in group B while 2 cases(13.33%) didn{u2019}t improve in group A compared to only one case (6.67%) in group B but only two cases worsen in group B (13.33%) compared to 3 cases (20%) in group A.As regard BCVA, in group A 8 cases (53.33%) was improved compared to 11 cases(73.33%) in group B while 4 cases(26.67%) didn{u2019}t improve in group A compared to 3 cases (20%) in group B. only one case worsen in group B(6.67%) compared to 3 cases (20%) in group A. Conclusion: Peeling ILM during PPV in DME was found beneficial than PPV alone both anatomically and functionally. PPV remains last resort in treatment of persistent DME and/or VMT ER -
