Background: Several sources of damage to the reticular dermis and subcutis may lead to cutaneous scarring, including burns, abrasions and surgery. In clinical practice many hypertrophic scars are difficult to manage. And so, this study was performed to evaluate the effect of shock wave therapy on patients with post-burn scars. Subjects: Forty patients were included in this study. Their ages ranged from 20 to 45 years, they were divided into two groups: Group (A) received shockwave therapy and traditional physical therapy, 2 days/week for 6 weeks. Group (B) received traditional physical therapy, 2 days/week for 6 weeks. Assessment: The measurements were done before the study and after 6 weeks of treatment for both groups by using Vancouver scar scale (grading system) and ultrasonography (for scar thickness). Results: The results showed that there was significant reduction in scar thickness with (p value=0.0001) and in total score of Vancouver scar scale with (p value=0.0001). Conclusion: Shock wave therapy is effective in managing immature post-burn scars
