Diabetes increases global health burden and form public health problem in Egypt due to increasing prevalence and disease complication. Therefore, apprising supply and demand sides is crucial to reduce diabetes disease burden in Egypt. Identify factors related to health system (supply), and patient behavior (demand) that predispose to occurrence of diabetes complications. The study was conducted in Kasr Alaini internal medicine hospitals (IMH). The study is health services operations research evaluation design. Study population included IMH inpatient with diabetes complications (cases) and outpatients in diabetes clinics (control). The number of cases (250) included all admissions to randomly selected two units (out of 6 units) in IMH within seven months. The control group (500) matched by age and sex with inpatients. A questionnaire form was used for structured interview with cases and controls. Diabetes was discovered before the age of 40 among 43% of cases and 34% of controls (p = 0.01 OR = 1.5 CI =1.08; 2.02). Major challenges for diabetes control strategies were stress management (80%). Their lifestyle issues with different control by cases compared to control were (p < 0.00) consuming fat and sugar free diet, physical exercises and regular drug intake
