The role of surgery and its timing remains undefined. The aim of this work was to clarify the surgical indication in these patients. Twenty patients with cerebral traumatic (IPH) were included. The patients were surgically treated early to minimize secondary brain injuries if the volume of hematoma exceeded 20cm3 with midline shift more than 5mm, basal cistern compression, increased intra cranial pressure (ICP) that not responding to medical treatment and neurological deficits related to hematoma, or any lesion more than 50 cm3 by either open craniotomy and evacuation under direct visual guidance for focal lesions in addition to decomressive crainectomy in cases associated with diffuse cerebral swelling that usually occurs secondary to the trauma ER -
