N2  - Pediatric fasting guidelines are designed to reduce patient discomfort while minimizing the risk of perioperative morbidities. This study aims to determine the current practice of pediatric preoperative fasting for day case surgery at Abu Elreesh children hospital; Cairo University, its effect on perioperative morbidities, awareness of surgery residents to the preoperative ASA fasting guidelines and adherence of parents to preoperative fasting instructions. In total, 200 parents and 16 surgery residents were participated in questionnaires, and a total of 200 children (2 months-12 years, ASA I or II) undergoing elective day case surgery under general anesthesia were enrolled in this prospective cross sectional study. We focused on the fasting duration, perioperative morbidities, parents understanding and compliance with fasting instruction as well as on the surgery resident familiarity and acceptance of new fasting guideline recommendations. Findings demonstrated that pediatric patients experienced prolonged fasting before procedures with no incidence of regurgitation or pulmonary aspiration. A total of 18.5% (n=37) parents believed that the fasting was necessary to avoid the development of preoperative health problems. Of the 18.8% (n=3) surgery residents were familiar with the new recommendations. Concerns regarding patient safety and every day work flow might explain the reasons against implementing new fasting guidelines
