Objective: To detect the accuracy of breast ultrasound in diagnosis of fibro adenoma and differentiate between typical and atypical ones. The impact of ultrasound criteria on patient management. (Biopsy versus follow up). Patient and methods: The study population included 52 patients (71 FAD) complaining of breast mass or pain, 32 of them have 35 Atypical FAD and 20 patients have 36 typical FAD by ultrasound. The 35 Atypical FAD. Seven of them were proved to be complex Fibroadenoma by biopsy and Twenty Eight were proved to be typical fibroadenoma. One of the patient with complex FAD, after 6 monthes interval follow up, it turn into malignant. Twenty patients have 36 typical FAD were followe up every six months for one year and were stationary course. Diagnosis was reached either by performing follow up study or after revision of core biopsy/ surgical specimens. Results: In our study, We have 35 FAD with atypical criteria by ultrasound seven of them (20%) proven to be complex fibroadenoma by pathology, Their mean age is (35.66±6.37) and range (23-41) and mean size (2.3±1.29), Mean age of the other patients (28 typical FAD) who proved to be typical fibroadenoma is (26.5±5.3), range (19-39), mean size 1.8 cm. We also have 20 other patients having 36 typical fibroadenomas by showing their typical criteria by ultrasound and by follow up, they have stationary course, with mean age 22.45±5.3, range 18-35, and mean size 1.8±1.6
