The present study was designed to compare two types of attachments, the ball and socket attachment versus the locator attachment used with complete mandibular overdenture retained with a single midline implant regarding; Patient satisfaction, gingival index, bleeding on probing, implant stability. This study was conducted on sixteen completely edentulous patients selected from the outpatient clinic of prosthodontic department, faculty of oral and dental medicine, Cairo University. The patients were divided into two equal groups: Group I patients received mandibular overdenture retained with single midline implant using locator attachment while Group II patients received mandibular overdenture retained with single midline implant using ball attachment. Patient satisfaction was evaluated using visual analogue scale one week, one month and three months after denture insertion. Clinical evaluation was carried out for implant stability using resonance frequency analysis immediately after implant insertion then after two months. Also gingival index and bleeding on probing were evaluated at denture insertion, after one month and three months. According to the results of this study, patient satisfaction showed an increase in both groups along the whole follow up period, also on comparing the two groups, there was statistically significant difference with higher values for the locator attachment group. The gingival index score values showed no statistically significant difference with time for both groups. Also comparing the two groups, there was no statistically significant change
