Background Colorectal cancer (CRC) is a disease predominantly affecting older individuals. Recent studies suggested a constantly rising incidence in young age. The aim of this study is to compare the clinicopathological features and survival of CRC in young with elderly patients. Methods This is a retrospective study that included all patients with pathologically proven adenocarcinoma CRC patients who presented to National Cancer Institute, Egypt between 2008 and 2012. Results Through this period, 607 patients were assessed. the mean age of the included patients was 49.1 ±14.3 years (ranging from 16 to 85 years) while the median was 50 years with slight female predominance. Twenty-nine percent of patients aged 40 years. Compared to the elderly groups, younger groups significantly had less co-morbidities, better ECOG performance status, higher incidence of left-sided colon cancers, higher pathological grade, more mucin-secreting histological type, more locally advanced tumors and more node-positive disease (P< 0.05 for all). The young groups had similar overall survival and event-free survival to their older counterparts. Conclusions There is increased incidence of CRC in young age and compared to elderly patients, the biological behavior of CRC in young patients is more unfavorable with more advanced stage at presentation. However, there was no significant impact of age on survival
