<br/><br/> The purpose of this randomized, double-blind, controlled study was to assess the effect of addition of Amoxicillin to AH plus  in single visit endodontics on  endodontic flare up in asymptomatic necrotic teeth. Forty-six patients were randomized and allocated to experimental and control groups where amoxicillin was added to AH plus in the experimental group while AH plus had been used alone in the control group. Flare up was considered as moderate-to-severe postoperative pain and/or moderate to severe swelling that began 12 to 48 hours after treatment and lasted at least 48 hours. The results demonstrated that 2.3% of the 46 patients had a flare-up. Of the five patients who had flare-ups,  2 were in the amoxicillin group and 3 were not. There was no statistically significant difference between both groups (P=1.000). We concluded that the addition of Amoxicillin to AH plus had no effect on flare-up in asymptomatic necrotic teeth
