Varicocele a common health problem affects adolescent and adults with an incidence of 15-20%. It is considered the most common treatable cause of infertility as it is associated with 35% and 69% of primary and secondary infertility cases respectively . DWI is one of the latest technology for providing information about tissue integrity, based on the movement of water molecules within different tissue compartments, the amount of restricted water molecules can be measured quantitatively and expressed as ADC. Diffusion of water molecules recognized to be restricted (decreased ADC value) in certain condition includes; inflammation, trauma, ischemia, tumor, and fibrosis. Thus, a decrease in the ADC values can be considered an indirect indicator of testicular damage
