ABO and Rh incompatibilities are common causes of blood group incompatibility in newborns. On rare occasions, alloimmunization due to minor erythrocyte antigens may cause severe hemolytic disease requiring exchange transfusion. Most common minor erythrocyte antigens include non-D Rh antigens (c, C, e, E), Kell, Duffy, Kidd and MNS. Patients and methods: One hundred and fifty newborns with indirect hyperbilirubinemia at NICU of Cairo University Children Hospitals. Analysis for minor blood group incompatibility (screening and identification) was done using ID Diapanel and ID Diapanel P system. Results: Our results showed 88 cases (58.7%) with no incompatibility, 39 (26%) cases with ABO incompatibility, 16 (10.7%) cases with Rh incompatibility, 5 (3.3%) cases with both ABO and Rh incompatibility and 2 (1.3%) cases with minor blood groups incompatibility. Conclusion: RBC alloantibodies against the minor RBC antigen can cause clinically significant hemolysis and it is being found more than previous days. So physicians should consider that minor blood group incompatibilities may be involved in the etiology of newborns who present with jaundice
