Introduction: Anterior cruciate ligament (ACL) is a distinct ligamentous which has been found to play an important role in the knee stability. Patients and methods: A randomized controlled trial was conducted on 30 patients who had ACL tear and grade 3 pivot shift. Each group had 15 patients; group (1) underwent anatomical single bundle ACL reconstruction with hamstring tendon graft while group (2) underwent anatomical single bundle ACL reconstruction with quadriceps tendon graft. Results: After 12 months follow up, there was no statistically significant difference between the 2 groups regarding Lysholm score, IKDC and the mean side to side difference of anterior tibial translation using KT1000. Conclusion: There is a statistically non-significant difference between using of hamstrings tendon graft in ACL reconstruction versus using of quadriceps tendon graft in ACL reconstruction .
