Background: Type 2 DM is worldwide major health problem .Its prevalence has increased in recent decades. Prediabetes is considered to be a risk factor for further development of type 2DM. There is a complex interplay between genetic, epigenetic and environmental factors in incidence of type 2DM. Objective: Our aim of this prospective study was to determine the prevalence of T2DM and prediabetes in first degree relatives of type2 DM patients. Methods: This cross-sectional study included 300 individuals who are first degree relatives to type 2 DM patients aged 20 years and above and not known to be diabetic. They were subjected to physical examination and had assessment of HbA1c and blood lipids. Results: In this study, diabetic individuals were 16% among FDR of T2DM and prediabetic individuals were 19.3%. It was noticed that 46,2 % of the newly discovered hyperglycemic cases had only a diabetic mother. By linear regression, there were four predictors of HbA1c levels. They were TG levels, SBP measurements, age and weight. Conclusion: The FDR to T2DM patients are a major risk for prediabetes and T2DM
